PlanStreet at the NHSDC 2026 Fall Conference

With the theme “Amplifying Outcomes With Data!”, the conference brings together HMIS teams, human services leaders, and service providers to explore how data can strengthen programs and improve responses to homelessness.

PlanStreet will showcase its Community-Based Care Platform, highlighting HMIS capabilities for coordinated entry, case management, shelter bed management, and HUD-validated APR and CAPER reporting. The focus will be on helping providers connect client records, coordinate housing and supportive services, and simplify reporting while keeping people at the center of every workflow.
