Organization Overview

VelNonArt Transformative Health serves communities throughout Los Angeles County through a range of community-based programs, including intensive case management, reentry support, mobile treatment access, harm reduction, workforce development, education, and expressive arts programming.

VelNonArt works with individuals experiencing overlapping challenges related to homelessness, mental health, substance use, and justice involvement. Their programs are designed to connect individuals with resources and services that support recovery, stability, and long-term independence.

Challenges Icon

The Challenge

VelNonArt's participants often require support across multiple areas of their lives. An individual may initially connect with the organization through community outreach, seek assistance accessing substance use treatment, and later need housing, employment, or reentry support.

Managing these needs requires staff to maintain accurate participant information, document interactions, and coordinate follow-up across different programs. Without a centralized record, keeping track of a participant's service history and ongoing needs can become more difficult as they engage with additional services.

VelNonArt needed a platform that could support its case management operations while providing staff with a consistent view of participant information and service history.

The Solution

PlanStreet serves as VelNonArt's centralized system of record for participant information and case management. The platform brings participant records, case documentation, and ongoing support activities together, helping staff maintain continuity as individuals engage with different programs and services.

Having a shared participant record allows staff to reference previous interactions, document progress, and coordinate support around each individual's needs and goals.

Implementation & Program Use Cases

VelNonArt has been using PlanStreet for several years to support its case management operations across multiple service areas.

Intensive Case Management & Reentry

VelNonArt's case managers work with participants to identify individual needs, establish goals, and connect with available resources. PlanStreet provides a central location for participant information and case documentation, helping staff maintain a history of services and ongoing support.

Housing, employment, and reentry needs can be documented within the broader context of each participant's case, supporting a more coordinated approach to service delivery.

Mobile MAT Access & Care Navigation

VelNonArt's mobile Medication-Assisted Treatment (MAT) teams include health navigators, peer support specialists, and case managers who help individuals access treatment and related resources.

PlanStreet supports the case management side of this work by maintaining participant information and service histories. This gives staff a central record to reference as participants navigate treatment access and receive ongoing support.

Harm Reduction & Community Outreach

VelNonArt provides harm reduction services, including overdose prevention education, supplies, screening connections, and community wellness events.

PlanStreet helps connect participant case management records with the broader context of community engagement. Staff can reference previous interactions and relevant participant information when coordinating follow-up and additional services.

Employment Training & Job Readiness

Through workforce development initiatives, VelNonArt provides Community Health Worker (CHW) training and opportunities for practical experience.

PlanStreet supports the documentation of employment and job readiness needs alongside other case management activities. This allows staff to consider workforce development as part of a participant's broader goals for stability and independence.

Education & Expressive Arts

VelNonArt also offers educational programs, organizational training, and expressive arts activities that encourage learning, personal development, and community engagement.

For participants receiving case management, PlanStreet provides a central record of their ongoing support, helping staff keep individual goals and service histories connected across different forms of engagement.

Coordinated Participant Support

A participant's engagement with VelNonArt may begin with an initial outreach interaction and develop into ongoing case management. For example, an outreach worker can document an initial contact and identify a participant's need for additional support. A case manager can then complete intake, document participant priorities, and establish a service plan. Treatment navigation, housing referrals, and other support activities can be documented as part of the ongoing case.

If the participant later enrolls in employment training, that involvement can be considered alongside their existing case management history. By maintaining a central participant record, PlanStreet gives staff a consistent place to reference previous interactions, review outstanding activities, and document progress as service needs change.
